- Abstract:
Preserving healthy pulp tissue and preventing apical periodontitis are at the core of Endodontology. Although, the dental pulp is able to recover from damage and severe irritation, traditionally treatment of exposed pulp has been unpredictable. Recent advances in pulp capping biomaterials, and improved understanding of pulp biology and regenerative processes have led to renewed enthusiasm for treating the exposed pulp; however, our understanding of the cellular regulators of pulpal inflammation/repair remains incomplete. Epigenetic modification of DNA-associated packing molecules and the influence of non-coding RNA have been shown to be central to several key processes in vital pulp treatment (VPT), including the self-renewal of stem cell populations and regulating mineralised tissue development and repair. Therapeutically, the use of pharmacological inhibitors targeted at epigenetic processes, such as DNA methyltransferases and histone deacetylases, have promoted repair processes in dental pulp cell (DPC) populations. This manuscript focuses on an analysis of the importance of epigenetic modifications on controlling DPC phenotype as well as the potential to target epigenetic modifications as part of a therapeutic strategy. Bevarelse af sundt pulpavæv og forebyggelse af apikal parodontitis står centralt i faget endodonti. Selv om pulpa er i stand til at komme sig efter skade eller kraftig irritation, har behandling af eksponeret pulpavæv traditionelt været uforudsigelig. Fremkomsten af nye biomaterialer til pulpaoverkapning har sammen med forøget indsigt i pulpabiologi og regenerative processer ført til ny optimisme vedrørende behandling af eksponeret pulpavæv; men vores viden om cellulær regulering af inflammation og heling i pulpa er stadig mangelfuld. Epigenetisk modulering af de molekyler, der indpakker DNAmolekylet, og virkningen af ikke-kodende RNA har vist sig at stå centralt i flere nøgleprocesser inden for vital pulpabehandling, herunder reproduktion af stamceller og regulering af mineraliserede vævs udvikling og heling. Terapeutisk anvendelse af farmakologiske hæmmere af epigenetiske processer, som fx DNA-metyltransferaser og histondeacetylaser, har fremmet helingsprocesser i pulpacellepopulationer. Denne artikel fokuserer på en analyse af betydningen af epigenetiske modifikationer for kontrol af pulpacellefænotyper og på mulighederne for at anvende epigenetiske modifikationer som led i en behandlingsstrategi.
